There are a variety of forms that students--in consultation with the faculty graduate advisor, major professor, and graduate program coordination--will need to complete during their academic careers. Most forms take 2-4 weeks for full processing, so please submit your forms as early as possible.
GradSphere Forms
These are the "smartest" forms available. They will be fully completed and approved within GradSphere. For any technical issues/questions, please email [email protected].
- Registration and Courses
Course registration, including adding and dropping courses, is handled via Schedule Builder. These forms are for students who wish to make changes to their registration after the deadline, apply for an alternative student status, and/or adjust variable units or grading for registered coursework. See the Graduate Studies Registration & Status and Grading, Courses, and Credit pages for more information.
Filing Fee**Also requires the Filing Fee Completion Form (PDF).
- Qualifying Exams
Information about the QE application process, committee eligibility, and exam outcomes is available in the Doctoral QE Policy and the Doctoral Qualifying Examination websites. Do note that QE applications are due to Graduate Studies at least 30 days prior to the expected exam date. To allow sufficient time for review, it is recommended that students submit this application at least 40 days in advance.

- Advancement to Candidacy
Candidacy is the phase of your graduate study at which you demonstrate preparation to begin work on your final requirement (thesis, dissertation, etc.) and select a committee for your graduate degree. For doctoral students, Candidacy is typically applied for after passing your QE; please refer to your degree requirements and contact your Graduate Program Coordinator, as Candidacy eligibility may vary based on approved degree plan.
- Committee Reconstitution
Change the members of a Ph.D. dissertation, Master's thesis or Qualifying Exam committee.
- Degree Completion
Degree completion workflows are those which can be used as you near the end of your degree progress and get ready to file to graduate. For further information on these processes, please contact your Graduate Program Coordinator.

PhD Final Exam Report (to be completed by Dissertaion Chair)
Docusign Forms
For the convenience of students and faculty, some of the more common forms students use have been converted to Docusign by the College of Biological Sciences team. Think of them as "semi-smart" forms! These forms will be completed in Docusign by the student and faculty approvers. The graduate program coordinator will then send the form to Graduate Studies for final approval and processing. For any technical issues/questions, please reach out to your graduate program coordinator.
Planned Educational Leave Request*
*Please make sure to discuss PELP requests with your major professor and advisor before starting the form.
PDF Forms
These less used forms are the "least smart." The student should complete the PDF and then send to their faculty graduate advisor and major professor (if applicable) for signatures. The final student step is sending the signed form to their graduate program coordinator who will complete their review before sending to Graduate Studies for final approval and processing. For any technical issues/questions, please reach out to your graduate program coordinator.
*Also requires the Filing Fee Application in GradSphere.
